How do you warm your car up when it's really cold

We've all had to get into a freezing cold car on a frosty and icy morning.

So how do you warm up your car?

Do you leave it running on the drive until the engine temp gets up? I hope not, that is really ineffective, damaging and potentially illegal in some locales.

Start engine, turn on fans/ac to max, scrape windows.
If I can see, slowly leave the driveway.
