What driving offence or annoyance winds you up the most

What driving offences or annoyances wind you up the most?

For me it's when I'm closely tailgated, or when people throw rubbish out of their car windows.

I'm usually quite tolerant when people do silly things as long as it's not putting other people at risk.

Did a 500 mile round trip this weekend - mostly motorways. The only two things that got my ire was tailgaters (if their light cluster is visible under my rear wing in the rear view mirror they are way too close) and drivers in the middle, or third, lanes when the left is clear.

I deal with the first by slowing a little then speeding up to increase the gap. Mostly this works, but the occasional dickhead will just come right back.
The second type I deal with by cruising by in the left lane. Again sometimes this works and the offending twat moves over.

Missed one. People that think national speed limit signs on single carriageways mean 50mph!!!
